Active Duty Exit Survey

In-Service Recruiters who participate in mass briefings for active duty separatees have the opportunity make a positive impression on behalf of both themselves and the Air Force Reserve Command. Just as importantly, the mass briefing is a great place to gather valuable information as part of the Investigate Step of IMPACT. The Active Duty Exit Survey, which can be downloaded from the link below, will help you collect a wide range of valuable information you can use to your advantage when you speak to each separatee on the phone or face-to-face. The file is prepared in WORD so that you can input your own contact information to personalize the form.   

Page 1 of the survey will gather contact information along with feedback from the separatee about where they are going, what they will be doing, what they enjoyed or didn’t enjoy about serving in the Air Force and whether there is already some knowledge of and interest in joining the Air Reserve. Having this information in your hands will help you ask the right questions during the Probe Step when you meet face-to-face for an appointment. Page 2 of the survey is designed for you to take notes in response to the separatee’s answers and provide you with a permanent record of your appointment. This information will assist you greatly as you prepare your recommendations for the Apply Step of IMPACT.
